PRIVATE PAY

Private pay options are available. The out-of-pocket cost of a pre-surgical evaluation is $800. This includes the clinical interview and feedback appointments, test materials, scoring and interpretation of results, and the final report. INSURANCE

Start Line Behavioral Health accepts health insurance for psychological evaluations and we are in-network with:

• Blue Cross Blue Shield

• Aetna

• Tufts

• Cigna

• Harvard Pilgrim

• Optum/United Behavioral Health

• Medicare/Medicaid

​

Out of Network: For other insurance providers, clients can request out-of-network provider documentation that they can submit to their insurance company for possible reimbursement. Start Line Behavioral Health, LLC is not responsible for out-of-network reimbursements to clients. Payment in full must be paid to Start Line at the time of service, and reimbursements are paid directly from the insurance company to the client.

48 hours notice is required for rescheduling or canceling appointments. If you reschedule or cancel any of the following types of appointments within 48 hours or do not show up for your scheduled appointment, you will be responsible for paying the late cancel fee of: ​

• $200 for the initial consultation appointment

• $100 for follow-up appointments

We accept credit or debit card payments. Payment for all fees, including insurance copays and deductibles, is due at the time of your appointment unless otherwise agreed upon. Card payments can be made online using our secure client portal.
