Fees for Emotional Support Animal Evaluations
The fee for an Online Emotional Support Animal (ESA) evaluation is $175, which includes the initial review of your online assessments, the 30-minute virtual evaluation, and any letters or documentation provided to you if an ESA is determined to be appropriate.
The full amount is due at the time of your scheduled appointment. When you schedule an appointment, you will be asked to add a debit or credit card to your secure client portal account. Start Line Behavioral Health, LLC will automatically charge the appointment fee to this payment method.
Fees for Therapy Sessions
Private pay is available for a fee of $200 for an initial psychiatric diagnostic evaluation and $175 for 50-minute individual psychotherapy sessions. All fees, including insurance copays, are due at the time of the scheduled appointment.
Start Line Behavioral Health accepts health insurance for therapy sessions and we are in-network with:
Blue Cross Blue Shield
Optum/United Behavioral Health
For other insurance providers, clients can request out-of-network provider documentation that they can submit to their insurance company for possible partial reimbursement. Start Line Behavioral Health, LLC is not responsible for out-of-network reimbursements to clients. Payment in full must be paid to Start Line at the time of service, and reimbursements are paid directly from the insurance company to the client.
24 hours notice is required for rescheduling and cancellations for all appointments. If you cancel any of the following types of appointments within 24 hours or do not show up for your scheduled appointment, you will be responsible for paying the late cancel fee of:
$175 for a therapy session
$200 for a consultation appointment
$500 for testing sessions
We accept cash, check, and credit or debit card payments. Payment is due at the time of your appointment unless otherwise agreed upon. Card payments can be made online using our secure client portal.
